AI-assisted development has a structural flaw that competent output tends to hide. The system rarely says, “I do not have enough guidance.” It proceeds: it fills gaps, decides what was meant, produces code or a plan, and reports success as if it had understood the task from the start.
This is the unreliable narrator problem. The model produces code, and alongside it a story—a claim to have understood, followed constraints, and met the goal. Sometimes that story is true. Sometimes it is only consistent with the model’s own assumptions. The danger is not constant failure. It is plausible success.
Anyone who has run AI agents on real work has seen the failures this produces. They appear in four familiar forms: the task is misread and the result solves a nearby problem; the agent works from the wrong context and duplicates or contradicts existing work; architectural judgment drifts even when constraints were stated; and the work is declared done before it is. These are ordinary failures, not exotic ones. What unifies them is not that narration causes them, but that it prevents the agent from detecting them on its own.
Absence becomes direction
Good human developers expose uncertainty. They ask, hesitate, and surface conflicts. Their doubt is visible and slows the work at the right moments.
AI tends to hide doubt. When guidance is thin, the model fills the gap: it invents requirements, chooses defaults, and builds structure around assumptions no one approved. Underspecification does not slow it down. It speeds it up.
A weak task can produce strong-looking output. A vague request can yield detailed code. A missing boundary can quietly become a new pattern in the codebase. Absence becomes direction—and that is worse than failure. Failure asks for correction. A plausible error asks for trust.
Why “hallucination” is the wrong word
“Hallucination” suggests obvious nonsense—output a competent reader would catch. The real danger is narrower and harder to see: local correctness with global error.
The code compiles. The names fit. The tests look reasonable. The explanation is fluent. Within the path the model chose, the work may be sound. But the path is wrong.
Fluency is not proof. Detail is not alignment. Confidence is not correctness. The system has solved a nearby problem and reported success from inside its own interpretation—possibly without knowing it guessed.
Why self-report cannot close the loop
When an agent reports done, it means less than it sounds: it has completed its version of the task. That version may be wrong.
The model judges its work against the same assumptions that produced it. If the task was misread, the report repeats the misreading. If success criteria were invented during execution, success is measured against the invention.
Self-report is circular. The actor cannot also be the witness. The agent that produced the work cannot be the authority that accepts it. This is why the failure mode in Article 1—open-loop execution—is so hard to catch from inside the loop.
In practice, this is not theoretical. A task can pass at the agent-report layer and still fail against the real contract. An agent will report success under a thin verification profile while a stronger fixture test exposes that the work does not match the contract. The useful signal does not come from the agent’s explanation. It comes from the external check that contradicts it.
What this implies for system design
Better prompts help. They do not solve this. The problem is not only weak prompts; it is that the model cannot reliably tell anyone, including itself, when its prompt was insufficient.
A dependable system must assume guidance will be incomplete—and that self-report cannot close the loop.
This leads to two architectural layers. The first constrains what the agent attempts: the task contract, scope, criteria, and forbidden patterns. The second decides whether the result should be accepted, using tests, contracts, and review criteria anchored outside the agent’s own story.
The frame narrows the space of plausible but wrong output before work begins. The gate refuses to accept output that fails verification, regardless of how confident the agent sounds. Both exist because the narrator cannot be trusted to detect its own drift.
Conclusion
AI systems do not fail only when they produce nonsense. They fail when they produce coherent work built on guessed intent and call it success.
That is the dangerous case, because it feels like progress.
